Request for Meningitis Immunization Extension

UHCL understands that students may have had extenuating circumstances preventing them from receiving the meningitis vaccination by the compliance date.

By law, under justifiable circumstances, an administrative official of the institution of higher education may grant extensions to individual students.

UHCL will consider extensions on a case-by-case basis for students who:

1. wish to register for the current term;
2. will receive the meningitis vaccination within 10 days of the compliance date or
3. will present the exemption documentation as required by law; and
4. have completed the Request for Extension Form.

Students submitting the extension request will be contacted by the Records Office within 1 business day.

Students will not be permitted to register until all required documentation has been received in the Records Office.

Students that do not apply or are denied an extension may still register 10 days after receiving the vaccination, provided registration is still open.
