On the UHS Portal page at https://my.uh.edu:

Click the "Forgot Your Password" link

Enter your EMPLID and click Continue...

Enter a response to the question in the response field.

Click Email New Password.

You should receive a new password at your work e-mail address within a couple of minutes.
