New Students

If you are transferring benefits to UHCL and you received VA benefits at another school, bring to Student Financial Aid:

Candidate Plan of Study (CPS) - contact your Advising Office. (A list of the academic advising offices is located on the left.)
Request of Change of Degree/Place of Training, VA form 22-1995
- Students receiving Dependent Benefits under Chapter 33 and Chapter 35 should complete the VA form 22-5495.
DD214, Member 4 Copy.
- Students receiving Dependent Benefits under Chapter 33 and Chapter 35 do not need to submit a DD214.
Chapter 33 applicants should complete a Chapter 33 Tuition Agreement Form.
A copy of your Military Transcripts
If you change your enrollment, complete the Enrollment/CreditHours Change form. In addition, students receiving Chapter 30, Chapter 1606, or Chapter 1607 benefits need to verify enrollment with VA once a month. You can do this online.

If you are a first-time applicant receiving benefits for VA Educational Benefits, bring to Student Financial Aid:

- If you are receiving Dependent Benefits under Chapter 33 or Chapter 35, please submit VA Form 22-5490, instead of the 22-1990.

- Students receiving Dependent Benefits under Chapter 33 and Chapter 35 do not need to submit a DD214.
