UNDERGRADUATE INTERNSHIP
GENERAL REQUIREMENTS (Starting Spring 2013)

Students enrolled in their final academic semester in Fitness and Human Performance (FHP) program at the University of Houston – Clear Lake (UHCL) are eligible to complete an elective internship course (HLTH 4739 or 6739).  Note that HLTH 4739 is only required for pre-clinical track students.  Students finishing all requirements of the internship course will receive 3 hours of academic credit.  In order to receive credit, students must carefully adhere to the guidelines listed below.  Failure to meet the FHP standard requirements will result in an inability to register for the course or receiving an “F” for your final grade.    

PRIOR TO ENROLLING IN THE INTERNSHIP COURSE, STUDENTS MUST:

  • Have completed all FHP core course requirements or be enrolled in their final semester of course work at UHCL.
  • Complete the UHCL internship application packet and receive advisor
    approval before enrolling.  The deadline for submission of the packet is the university graduation deadline of the academic semester prior to enrolling in the internship.
  • Develop a letter of application and resume that is reviewed by the UHCL career services.   The resume must be approved by the FHP field supervisor prior to submitting to the internship agency.
  • Submit documentation showing a background check was administered by the UHCL police department and a drug screening by the UHCL health services.  The costs associated with the background check and drug screen is solely the responsibility of the student. 

WITHIN THE FIRST WEEK OF THE INTERNSHIP SEMESTER STUDENTS MUST:

  • Submit a signed copy of your “Statement of Agreement” to the UHCL field supervisor.
  • Submit a completed learning contract to the agency and UHCL site supervisor. 
  • Submit an estimated weekly schedule approved by the internship agency to the UHCL field supervisor.

PRIOR TO MID-TERM STUDENTS SHOULD:

  • Submit a summary report detailing the first 8-week experience.
  • A one-page description of the proposed project, agreed upon by the site supervisor.
  • Schedule a site visit with the UHCL site supervisor. 

PRIOR TO COMPLETION OF THE INTERNSHIP, STUDENTS SHOULD:

  • Complete 150+ hours, with signed weekly logs verified by the intern supervisor. Logs should be kept on the UHCL timesheet provided by the field supervisor. 
  • Complete your personal project.  The project is in addition to your normal intern duties.  Both the FHP intern supervisor and the on-site intern supervisor must approve this project.  
  • Develop a 3-5 minute presentation detailing the internship experience.  The report should include information about the cooperative agency, your project, and the internship experience.
  • Completed Student Intern Evaluation form sent back to the FHP Internship supervisor by the on-site internship supervisor.  This can be mailed, e-mailed, or included in the student’s notebook.
  • Obtain a letter of recommendation from the Agency supervisor.
  • Have completed a site visit with the University supervisor.
  • Hand deliver or mail a verified (initialed by your Agency intern supervisor) typed log of hours and activities each week of the semester to your FHP intern supervisor.  You must also prepare a notebook containing the all pertinent internship information.  The notebook should include a table of contents, dividers any pictures, handouts, charts, etc. related to your internship experience.
  • Complete the FHP exit exam, scoring 70% or higher.
