The University of Houston - Clear Lake uses the same forms as the University of Houston System.  In the navigation bar on the bottom left there is a link called UNIVERSITY OF HOUSTON SYSTEM FORMS.  Click the link and it will take you directly to the FORMS site.  Scroll down for Animal Care forms.  

To access the animal care forms via University of Houston's Division of Research website, go to http://www.uh.edu/research/ .  Click DIVISION OF RESEARCH (link found on left), then TOOLBOX (links found on either the left or right), and FORMS (link found on far right). Scroll down to ANIMAL CARE FORMS.  Use the ANIMAL CARE APPLICATION FORMS for full application of a new protocol or upon third year of renewal.  Use the ANIMAL CARE RENEWAL/ADDENDUM FORMS for annual renewal reviews of years 1 and 2 or for addendum(da).   

We recommend selecting the "Word format" and saving the form to your hard-drive for completion and later submission.  Submit completed forms to the Office of Sponsored Programs via Roberta Hohmann (hohmann@uhcl.edu).
