All necessary forms and directions for applying to the Committee for the Protection of Human Subjects (CPHS) are linked in the navigation bar to the left.  The application form is entirely self-explanatory.  It contains boxes and directions for filling out the necessary information.  It also contains the regulations that apply to the majority of cases here at UHCL.  Even if you think your project is exempt from CPHS review, only someone on the committee is empowered to make that decision.  The forms open as Word documents and we recommend that you save them to your own hard drive for completion and later submission.

The CPHS is comprised of members from each of the four schools and includes alternates in case the primary member is not present.  Other members include external and non-research individuals.  When submitting your application, submit to your school representative and send a copy to the the Office of Sponsored Programs via hohmann@uhcl.edu.
