Required Documentation:

1. Completed UHCL Scholarship Application Cover Sheet (below)

2. A brief statement (1-2 paragraphs) to include:

• a) your job title and department;
• b) objective for taking class;
• c) a list of your PASA participation within the past 2 years and/or ideas you have to increase your future PASA participation

Two (2) copies of the application packet must be turned in to Office of Student Financial Aid Box 5 / SSCB 1.105.

Must be received by December 12, 2008 at 5:00pm.
