You will need each of these forms at some point during the thesis process.

	Master's Option Course Enrollment Form - PDF
Use this form to register for SWEN6939 after your proposal defense is approved. Some of the fields allow you to type information online. Then you can print the form and obtain all required signatures. Committee Approval Form A sample of this form is displayed on the Samples page. Use this form to submit to the Dean and Associate Dean for committee approval. Remember, it is your responsibility to obtain all signatures.
