Student Request for Alternative Texts

Let us know the alternative texts you require.

Before requesting alternative textbooks, you must have registered for your classes using E-Services and with Disability Services as either a new student requesting accommodation services or as a continuing student.

Requests are handled on a first-come, first-served basis, so send us your requests as early as possible.

Your alternative text options are pdf, eBook, doc/docx,txt and other.
Numerically rank these options which are listed below with the number 1 (one) indicating your most preferred format:

Textbook information

Up to 10 textbook requests can be made with this form. For each textbook request, you are asked for the class and ISBN. The textbook title is optional. At the end of a textbook request, you can Add another Textbook or Remove Last Textbook Request.

Submitting This Form
  1. A Thank You web page is displayed when your form is sent successfully. If you do not see a Thank You page, your form was not sent.
  2. You can expect an email confirmation from Disability Services within 1-2 business days.
  3. If you have any questions, comments, or problems with this form, call us at 281-283-2648.
