Use this form to schedule an online tutoring appointment. To schedule an in-person appointment, visit https://therichco.com/wconline/uhcl/schedule/ .
You will see instructions on how to e-mail your Microsoft Word .doc file on the 'thank you' page that is displayed after you submit this form. Fill this form out entirely, as incomplete forms may delay setting up your appointment.
You should see a Thank You message displayed.
If you do not, your form was not sent.
You will be contacted within 2 business days using your preferred contact information. Only at that time will your appointment be scheduled.
